Crisis Home Treatment

The Crisis Home Treatment Service works directly with people who are experiencing a severe mental health crisis and need immediate, urgent or emergency care and treatment in their home.

 

What services do we offer?

Our Crisis Home Treatment Team consists of the most senior staff within acute mental health and focuses upon mental health assessment. This can take place in the individual’s own home, community, or hospital settings.

While the service does not take an individual on a long-term basis, it does support them and their families during the acute phase of an individual’s mental health problems. The team can also make the necessary referrals to other services if required by working closely with the service user and family to find the most suited form of treatment.

The team has direct access to inpatient beds and other community services, as well as medical, nursing and social care input and advice.

HOW DO I ACCESS THE CRISIS HOME TREATMENT SERVICE?

The Crisis Home Treatment Service provides an open referral system where people can self-refer via the Single Point of Access.

They can help you if you or a loved one are experiencing a severe mental health crisis and need immediate, urgent or emergency care and treatment at home.

The service can be contacted 24 hours a day, seven days a week.

We welcome referrals to our services from healthcare professionals and GPs via our Single Point of Access.
