Referrals to Community Forensic service are through local services or criminal justice agencies.
Forensic Service referrals are made through local psychiatric services and the criminal justice agencies: criminal justice liaison – at local police stations, magistrates’ court and the probation service; community – advice and liaison to primary care, community mental health teams and voluntary organisations; secure units and special hospitals – private care services in the community; prison in reach and aftercare – at remand and sentence sites.
Internal requests for community forensic joint working should be discussed initially by speaking with a member of the team on (01472) 252366 or emailing request to email@example.com and then discussed at our team meeting which takes place on a Wednesday afternoon.
You can contact the team directly to arrange an informal telephone consultation. The team will be able to discuss a potential referral or advise on other options. A referral coordinator is available Monday – Friday between 9am-5pm.
Referrals to the team can also come from many different areas of the community such as other health agencies, school and colleges as well as other young people’s services. Your GP can also refer you.

Referrals to Liaison and Diversion are from the criminal justice system via an out out screening process at the point of access.
The Criminal Justice system is able to make a referral directly to the team via our main office number on (01472) 808545 or via email at firstname.lastname@example.org
Referrals for NAViGO Extra are made directly to the service.

Open Minds encourages self-referral which means to make an appointment you can simply call the team or walk into our Grimsby or Cleethorpes branch. If you would prefer you can also ask your GP or health professional to refer you.

You can refer between the hours of 9.30am and 4.30pm Monday – Friday, expect for Tuesday when it is 9.30 – 3pm due to staff meetings. Telephone referrals are also taken between these times.
Appointments are available between 9am – 5pm on Monday – Friday. On Tuesday and Wednesday we offer limited appointments between 5pm – 8pm

Referrals for all Rharian Fields Services are made directly to the unit.
To increase the accessibility of our Eating Disorder Service, we accept referrals made by any health or social care professional.
Referral forms for all our services can be accessed here.
All relevant information is required before a referral will be considered. Along with a fully completed referral form we will also require current blood test results and a recent ECG. This information enables our team to make an informed decision about the risks associated with each individual case and facilitates us to implement the most appropriate care package in a timely manner.
All referrals will be reviewed in relation to the seriousness of the individual case either on the same day or the following morning. Referrals received over a weekend will be reviewed first thing on the Monday morning by a Senior Eating Disorder Practitioner.

Referrals to the service can be made by your GP, care co-ordinators, employment agencies or by self-referral.
The process begins with the individual completing an expression of interest form. On completion an appointment to discuss the training, work experience and other opportunities that may benefit the individual will be arranged. If you are in need of immediate help you can contact the Single Point of Access, the local mental health crisis support service.
The service can be contacted 24 hours a day, 7 days a week.
The Crisis Home Treatment Service provides an open referral system where people can self-refer and ask for help. This is done via the Single Point of Access.
